Global Reunion Reservation

Please fill out the following reservation/order form in its entirety AS SOON AS POSSIBLE so that we can have a reasonably accurate count of how many people we can expect to attend.  We urge you to fill out the form even if you expect to get your tickets directly from one of the Comittee members as it will make our accounting much easier.

Enter the last name which will appear in the attendee list. We recommend using the name you used while attending BHS.
Enter your maiden name if you used your married name in the last field.
Name (if different) that will appear on the check you will use to pay for tickets. This name will not appear anywhere publicly, but it will allow us to credit your payment to the right person.
Enter the year you graduated (or would have graduated) from BHS. This is generally the year you wrote O' Levels.

If you will pickup your tickets from one of the committee members, please let us know who you would like to get them from and how they may contact you. Otherwise, enter your mailing address below.
Enter the address where you would like to have your tickets mailed. If you'd like the tickets to be mailed to someone other that the payment name above, include the name at the top of the address.
If purchasing more than one ticket, enter a comma separated list of the names of other people who will be attending.
